Prior to your initial appointment, please fill in the Intake form below and either email us or fax to (310)694-3475 (please disregard the old # on the form).
If you are not able to email or fax, simply bring the form with you to your appointment.
 
 
 
 
Please allow approximately an hour and half to Two hours for your first visit
and one hour to  an hour and half for subsequent visits.
 
Payment Options:
Payment in the form of Cash or Check is collected at the time services are rendered.
We don't accept credit or debit cards at the office.
 
Cancellation Policy:
We kindly request at least 24 hours advance notice to reschedule or cancel an appointment.

Important Note:
We are no longer at 1611 Montana Ave. We have recently moved to just a couple of blocks away from there. Still in Santa Monica with easy access from nearby cities of: Brentwood, Venice, West Los Angeles, Pacific Palisades, Westwood, Marina Del Rey, Culver City, Beverly Hills, Century City, Bel Air and West Hollywood.
